Promoting Health Through Community Collaborations


To Our Children's Future With Health’s PHCC Division ensures the participating youth and families in the Haddington & Nicetown-Tioga areas of Philadelphia receive an integrated continuum of care by providing coordination of preventive health and primary care services.

 

o       Preventive Health Services- Caries out the agency mandate that all youth enrolled in any program supported by To Our Children's Future With Health maintain current health assessments.  Through our partnering physicians, Preventive Health Services connects youth with their primary care physician and provides youth enrolled in any of the programs supported by To Our Children's Future With Health access to a primary care physician at no cost to them.

o       Primary Care- Youth participants who receive an initial physical and immunization through TOCFWH are eligible for sick and continuum care and health education.

o        HIV Prevention- Addresses the prevalence of HIV/AIDS in the Haddington and Nicetown-Tioga communities through awareness and education to all participating youth. HIV testing, counseling, and treatment is strongly encouraged for youth ages 15 to 18.

o        Research Evaluation- Assesses the effectiveness of the continuity of healthcare services offered to participating youth, using an internal database and well-documented best-practices in promoting community health, as well as parent surveys.
